Mistakes by medical staff have been identified after a pregnant woman died of blood loss following childbirth, an investigation panel has reported.
But the panel, formed to probe the blunder at Queen Elizabeth Hospital in Yau Ma Tei, failed to mention whether any personnel would be penalised for the incident in October.
